  • The social worker collects all documents related to the patient and approaches the different organizations that provide cardiac rehab services.
  • According to the constraints, patient's family and the social worker opts the best suitable option of a rehab center for the patient
  • By doing the necessary documentation, admission of the patient is confirmed in the rehab center.
  • After the confirmation, the rehab personnel which include a psychologist, a rehab expert and a cardiac exercise expert approach the patient in hospital for his phase 1 of rehabilitation.
  • When the Phase 1 of cardiac rehab is complete and the cardiac reports are quite normal then the patient is discharged from the hospital.
  • Following the discharge, the patient either has to stay in the rehab center or visit the center time to time for his Phase 2 rehab
  • In Phase 2, the rehab procedure includes a blend of cardiac exercises, meditation, and other medicines along with stress release and control technique applications
  • After the Phase 2, Phase 3 starts. In this phase, more emphasis is given on the cardiac exercise which helps to improve the blood pumping ability of the patient's heart. This helps in successfully dealing with mental and physical stress situations in his normal life.

  • After the Phase 3 of cardiac rehab is over, the patient is free to live his normal life. However, he has to strictly follow some rules and hence avoid getting another heart complication.
